ETD Forms

PRIOR to submitting the required forms to the Clearance Advisor, students should make copies of the paperwork for their records. The Office of Graduate Studies will NOT make copies of forms for students.

All signatures must be original. Also, all signatures must be obtained PRIOR to submitting the forms to the Clearance Advisor.

FSU will no longer use your Social Security Number on important forms or documents. Instead, a personalized "FSUSN" will be assigned to you. In order to retrieve your FSUSN, you must log onto Blackboard. On the left side of the screen you will see a "Tools" link. Find, "FSUID" and click it. You will see "Quicklinks" on the left side of the screen. Click "See Your FSUSN." You will be given two prompts on how you are able to retrieve your FSUSN. You can either use your FSUID username and password. Or, you can use your FSUCard Number and your Social Security Number. When prompted, you must use this number on all required forms.

Defense Announcement - PDF

  • Note: Unless you own the full version of Adobe Acrobat, you will be unable to save your "fillable" Defense Announcement as a PDF, and therefore unable to send the completed Defense Announcement to the Clearance Advisor. Instead it is recommended that you save your completed Defense Announcement as a JPEG or TIFF file. Once saved as a JPEG or TIFF file, you will be able to e-mail your completed Defense Announcement to the Clearance Advisor without issue.

Outside Committee Member Dissertation Defense Report - PDF

  • Note: All Outside Committee Members of Doctoral Students MUST complete and return this form to the Office of Graduate Studies no later than two-weeks after the successful defense of your students. Submission of this form is required in order for your student to be cleared for graduation.
